Job summary

AdvisaCare is seeking a compassionate Hospice Registered Nurse (RN) for part-time/per diem work in the Portage, Michigan area. The role focuses on in-home end-of-life care with an emphasis on presence, education, and emotional support for patients and families.

The ideal candidate holds an active Michigan RN license, has hospice/home health experience, and can work independently with excellent organizational skills.

Qualifications

• Current Michigan RN license in good standing.
• Minimum 1 year of hospice experience.
• Empathetic approach to patients and families.
• Ability to participate in field visits daily and in weekly office meetings.
• Ability to adapt to patient status and needs on a full-time on-call basis.

Responsibilities

• Initial and ongoing assessment of patient needs (physical, functional, psychosocial, environmental).
• Implement and revise the plan of care as needed.
• Ensure compliance with AdvisaCare Hospice policies and Medicare requirements.
• Educate patient and family on disease process and end-of-life care.
• Supervise paraprofessionals providing services to patients.
• Prepare clinical/progress notes and participate in IDG conferences.
• Contribute to Hospice performance improvement initiatives.
• Maintain high levels of customer satisfaction with clients, families, and caregivers.
