Job summary

Enhabit Home Health & Hospice in Alabama seeks an experienced Registered Nurse (RN) to coordinate care and implement interdisciplinary plans for home-based patients. You will assess patient and family needs, follow physician orders, and ensure care complies with federal and state regulations.

Requirements include graduation from an approved nursing program, an active RN license in the state, at least 1 year of RN experience, and basic tech skills including mobile devices.

Qualifications

• Must be a graduate of an approved school of nursing education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience as a licensed registered nurse (RN).
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Provide coordination of care, ensure continuous assessment of each patient's and family's needs, and implement the interdisciplinary care plan.
• Perform services in accordance with the physician’s orders and the established plan of care (POC).
• Ensure delivery of care is compliant with the federal and state regulations.
