Job summary

VITAS Healthcare seeks an experienced home care RN to ensure hospice patients’ comfort and build compassionate connections with families in their homes. You will guide patients and families through the end-of-life journey with autonomy in bedside care decisions, emphasizing comfort and dignity.

Daily work involves caring for patients in private residences or care facilities, with 4–6 visits per day and weekly interdisciplinary team meetings to coordinate holistic support.

Qualifications

• RN license required.
• At least two years of nursing experience.
• Reliable transportation.
• Commitment, compassion and a can-do attitude are valued.

Responsibilities

• Provide end-of-life hospice nursing care to patients in home settings.
• Coordinate with an interdisciplinary team, including social workers, physicians, chaplains and nurses, during weekly visits.
