Job summary

UT Medical Center Home Care Services in Knoxville seeks an RN to deliver and direct home health nursing care in the community. The RN will coordinate care plans with physicians and families, ensuring compliance with state rules and agency policies.

The role involves initial assessments, plan development, documentation, and high-quality clinical care from admission to discharge while meeting payer guidelines.

Qualifications

• One year of clinical RN experience.
• Current RN license in the state where services are rendered.
• Current CPR certification, driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation.
• Must be able to reliably commute to multiple patient residences daily within the geographic area assigned.

Responsibilities

• Provide and direct nursing care to patients in their homes as prescribed by the physician and in compliance with laws and agency policies.
• Coordinate the total plan of care with other health care professionals and maintain continuity of patient care.
• Make initial/comprehensive nursing evaluations, develop plans of care, and ensure home health eligibility and medical necessity.
• Maintain and submit accurate documentation and clinical notes from admit through discharge.
