Job summary

Personal Touch Home Health Care Services Inc. seeks a compassionate Hospice RN to provide end‑of‑life care in patients’ homes and community settings. You will assess patients, manage pain and symptoms, and implement individualized care plans with an interdisciplinary team.

In this role, you will support patients and families through transitions, document activities in the EHR, and participate in on‑call coverage as needed. A Missouri RN license and hospice experience are essential.

Qualifications

• RN licensure in Missouri and clinical expertise in hospice/palliative care.
• Strong assessment and symptom management skills.
• Ability to develop individualized care plans and provide patient-centered hospice care.
• Excellent communication and empathetic bedside manner.
