Job summary

You will coordinate symptom management, educate families, and collaborate with a multidisciplinary team to ensure seamless end-of-life support.

The role requires an RN with a Bachelor's degree, hospice certification preferred, and at least 1 year of supervised RN experience.

Qualifications

• RN with Bachelor's degree in Nursing.
• Certification in Hospice Palliative Care Nurse and Critical Care preferred.
• 1+ year of supervised clinical experience as a registered nurse.
• Excellent communication skills.
• Strong computer skills.
• Excellent organizational and interpersonal skills.

Responsibilities

• Assess patients with advanced illnesses and create individualized hospice care plans.
• Provide education, emotional support, and guidance to patients and families.
• Collaborate with VNA clinicians and external providers for seamless care.
• Document patient status updates and adjust care plans as needs evolve.
