Qualifications

• RN with a Bachelor’s degree in Nursing.
• Hospice Palliative Care Nurse and Critical Care certifications preferred.
• 1+ year supervised clinical RN experience.
• Excellent communication, strong computer, and organizational skills.
• Interpersonal skills and team collaboration.

Responsibilities

• Complete a two-week orientation focused on hospice care practices, EMR systems, and safety protocols.
• Assess patients with advanced illnesses and create individualized hospice care plans.
• Provide education, emotional support, and guidance to patients and families as they navigate the end-of-life journey.
• Collaborate closely with VNA clinicians and external providers to ensure seamless, compassionate care.
• Document patient status updates and adjust care plans as needs and goals evolve.
