Qualifications

• GA RN license active and in good standing.
• 1–2 years of nursing experience; hospice, palliative, or home health preferred.
• Strong knowledge of pain management, symptoms control, and end-of-life care protocols.
• Excellent communication and interpersonal skills with patients, families, and care teams.

Responsibilities

• Provide skilled nursing assessments and direct patient care to hospice patients at home or in facility settings.
• Develop, implement, and update individualized patient care plans with the interdisciplinary team.
• Manage pain and symptom control to maximize comfort and quality of life.
• Educate and support patients and families on disease progression, medications, and end-of-life care.
• Coordinate with physicians, social workers, chaplains, and aides to ensure holistic care.
• Accurately document patient assessments, care plans, and clinical notes per hospice regulations.
