Job summary

Aura One Hospice in Palm Desert, CA is seeking a compassionate Certified Home Health Aide (CHHA) to join our team. The role provides in-home personal care, mobility support, and end-of-life comfort aligned with hospice standards.

Candidates must hold a CA CHHA certification, CPR/First Aid, and a valid driver’s license. Experience in hospice or home health is preferred; travel and independent work are required. Flexible work-from-home options are available where appropriate.

Qualifications

• Prior hospice/home health/palliative care experience preferred.
• Current CPR/First Aid certification.
• Reliable transportation and valid driver's license.
• Strong interpersonal skills with a compassionate, patient-focused demeanor.
• Ability to work independently and as part of a collaborative care team.
• Comfortable working in home environments with end-of-life care needs.

• Provide personal care to hospice patients in the home setting, including bathing, grooming, and hygiene assistance.
• Assist patients with mobility, transfers and ambulation per care plan.
• Monitor and document patient condition changes and report to supervising nurse.
• Support patients with light housekeeping, meal prep, and ADLs.
• Deliver compassionate, patient-centered care aligned with hospice philosophy.
• Communicate effectively with interdisciplinary hospice team, patients, and families.
• Maintain accurate documentation and adhere to care plan guidelines.
