Job summary

Aura One Hospice in Palm Desert, CA is seeking a compassionate Licensed Practical Nurse (LPN/LVN) to join our dedicated care team. In this role you will provide expert clinical care and emotional support to patients and families at end of life, following the state Nurse Practice Act and the established plan of care.

The ideal candidate is a graduate of an accredited LPN/LVN program, holds California licensure, has at least three years of nursing experience (hospice or med/surg preferred), CPR

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• Three years nursing experience; hospice or medical/surgical experience preferred.
• Currently licensed as an LPN/LVN in California.
• Possesses and maintains CPR certification.
• Must have a valid driver’s license with insured vehicle.
• Strong verbal and written communication and organization skills.
• Understands hospice philosophy and needs of the terminally ill.

Responsibilities

• Provides direct patient care as defined in State Nurse Practice Act, including end-of-life care.
• Implements current nursing practice following assessment and the plan of care.
• Provides accurate and timely documentation reflecting the plan of care.
• Educates patients and family/caregivers relevant to diagnosis and care plan.
• Coordinates hospice services with interdisciplinary team and reports needs.
• Uses and prepares equipment following aseptic technique.
• Manages pain and symptoms and evaluates treatment responses.
• Engages in professional growth and quality improvement activities.
• Performs other duties as assigned by the RN.

About the Role:

Aura One Hospice is looking for a compassionate and dedicated Registered Nurse to join our team in Palm Desert, CA. In this meaningful role, you'll provide expert clinical care and emotional support to patients and families during life's most important moments. Responsibilities:

1. Provides direct patient care as defined in State Nurse Practice Act, including specialized duties related to end of life care.
2. lmimplements current nursing practice following a comprehensive assessment and the plan of care.
3. Provide accurate and timely documentation of patient services to reflect the plan of care.
4. Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self plan of care.
5. Participates in coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group. Examples include hospice aide, OT, PT, SW, ST, Dietician or Clinical Supervisor.
6. Uses and prepares equipment and supplies effectively, adhering to aseptic technique
7. Provides appropriate pain/symptom management. Evaluates patient's response to treatments/med ications.
8. Participates in personal, professionalgrowth and development. Also participates in organization's quality assessment performance improvement teams and activities.
9. Requirements:

1. Graduate of an accredited practical nurse or vocational nursing program.
2. Three years nursing experience. Community health/hospice or medical/surgical experience is preferred.
3. Currently licensed as an LPN/LVN in California.
4. Complies with accepted professional standards and practice.
5. Demonstrates good verbaland written communication, and organization skills.
6. Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
7. Possesses and maintains current CPR certification.
8. Understands philosophy of hospice concept. Also understands needs of the terminally ill.
