Job summary

Harmony Hospice in Chicopee, MA seeks an experienced Hospice RN to join our compassionate team. The RN will plan, coordinate, and provide hospice care aligned with the patient’s plan of care, partnering with an interdisciplinary group to address physical, emotional, and spiritual needs.

Qualifications

• Graduate from an accredited School of Nursing.
• Licensed in the State of Massachusetts.
• One year of RN experience; Hospice experience preferred.

Responsibilities

• Coordinate hospice care with the IDG and family to ensure needs are met.
• Evaluate the effectiveness of hospice services for patient and family.
• Deliver professional nursing care using the nursing process.
• Consult with attending physicians and Hospice Medical Directors on patient condition changes.
• Submit complete, accurate clinical notes in a timely manner.
• Participate in IDG and case conferences.
• Develop and implement the initial plan of care with IDG.
