Trinity Health At Home is seeking a Hospice RN Case Manager to provide professional nursing care to hospice clients, as prescribed by the physician and in accordance with licensure and agency policies. The role includes assessment, care planning, intervention, and supervision of care outcomes, with a focus on comfort and quality of life for patients and families.

The position offers a Contingent/PRN schedule of 0-20 hours per week, including two weekend days per month (Sat/Sun) or two Sat shifts

Qualifications

• Must have graduated from an approved nursing education program.
• Current RN licensure in the state of practice (Michigan).
• Minimum of one year experience as a professional acute care nurse.

Responsabilités

• Provides professional nursing care to hospice clients as prescribed by the physician and in accordance with licensure and agency policies.
• Develops and implements an individualized Plan of Care focused on improving quality of life and dignity.
• Documents observations, interventions and outcomes meticulously in the patient record.
• Collaborates with physicians and interdisciplinary team to coordinate care.
