Job summary

Trinity Health At Home is seeking a Hospice RN Case Manager in Michigan for a contingent/PRN role (0–20 hrs/week) with two weekend days per month. The RN will assess, plan, and coordinate hospice care, collaborating with physicians and the IDT to manage symptoms and support patients and families.

The position emphasizes grief/loss support, documentation, and adherence to nursing standards. Requires active RN licensure in the state and a valid driver's license.

Qualifications

• Graduate of an approved Nursing education program.
• Current Registered Nurse licensure in the state of practice.
• Minimum of one year experience as a professional acute care nurse.
• Home healthcare/hospice experience preferred.
• Exhibits strong communication and interpersonal skills, with emphasis on grief/loss management.
• Must have current Driver’s license and reliable transportation to/from work site.
• Commitment to the mission and Ethics/Compliance program.

Responsibilities

• Informs clients and caregivers about care/services and Hospice benefits.
• Develops and implements an individualized Plan of Care focused on outcomes.
• Coordinates care and referrals with other disciplines and services.
• Performs nursing assessments and provides clinical judgement.
• Reports changes in client condition to physician and updates plan of care.
• Re-evaluates and updates patient care plan based on goals and progress.
• Educates patients and families on disease process, self-care, and end-of-life care.
• Documents all care activities accurately in the electronic record.
