Job summary

Alabama Hospice of the Shoals, part of the LHC Group and Optum family, seeks a Registered Nurse Case Manager to oversee hospice care plans and quality outcomes. The PRN role covers weekday and weekend shifts, delivering compassionate nursing to patients and families in the community.

You will evaluate eligibility, coordinate care, maintain detailed clinical notes, and work with the IDG to ensure excellent patient outcomes. A strong nursing license and CPR are required.

Qualifications

• Current unrestricted RN licensure in Alabama.
• CPR certification current and valid.
• Reliable transportation or access to a dependable vehicle.

Responsibilities

• Makes the initial nursing evaluation within 48 hours of referral.
• Identifies patient/family needs and reassesses every 15 days.
• Initiates and coordinates the plan of care with accurate clinical notes.
• Documents problems, goals, interventions and responses to care.
• Collaborates daily with the patient/family, attending physician and IDG.
