Qualifications

• Graduate of an accredited nursing program.
• Current RN license in the state or valid multistate compact license.
• CPR certification and valid driver’s license required.
• Hospice/palliative care experience preferred.
• Ability to work independently while collaborating with an interdisciplinary team.

Responsibilities

• Complete initial and ongoing patient assessments across physical, emotional, psychosocial, and environmental needs.
• Coordinate with physicians for certification and recertification of terminal illness per Medicare regulations.
• Serve as the clinical point person for assigned patients, including symptom escalation and advanced care planning.
• Collaborate with providers and support systems to ensure seamless hospice care.
• Develop and update individualized care plans with patients, families, and the IDG.
• Provide skilled nursing care and document interventions and outcomes accurately.
