Qualifications

• Minimum one (1) year of hospice, palliative care, or home health experience preferred.
• Knowledge of hospice regulations and Medicare Conditions of Participation preferred.
• Ability to work independently while collaborating across interdisciplinary teams.

Responsibilities

• Conduct comprehensive nursing assessments and care planning.
• Coordinate with physicians for patient care documentation.
• Educate patients and families on hospice philosophy and symptom management.
