Job summary
HealthPartners is seeking a Registered Nurse to join our Hospice team in Bloomington, MN. You will provide end-of-life care in patients' homes or community settings, collaborating with an interdisciplinary team to deliver compassionate, patient- and family-centered care.
The ideal candidate holds MN RN licensure, has 2 years of recent nursing experience, and can work independently. A valid MN driver’s license and reliable vehicle are required to meet patient needs. Fluency in English required.
Qualifications
- Graduate of accredited nursing program.
- Current RN licensure in Minnesota.
- Two years recent nursing experience in acute, long-term care, or home/hospice/public health.
- Ability to work independently and communicate effectively.
- Current MN Driver’s License with access to reliable vehicle.
- Computer literacy.
Responsibilities
- Provide end-of-life care in patients' homes or community settings.
- Develop and implement individualized plans of care.
- Coordinate with the interdisciplinary team to optimize patient outcomes.
- Educate patients and families on care plans and symptom management.
- Participate in weekend and evening/night call schedules.
- Contribute to Quality Improvement initiatives.
