Qualifications

Responsibilities

• Coordinate and provide hospice care for home care patients in collaboration with patient and family.
• Develop, initiate and update a plan of care with patient and family.
• Oversee the interdisciplinary team and educate patients and families about disease, symptoms and prognosis.
• Document visits and update plans of care at point of care.
• Provide support and strength to patients and families during difficult times.
