Job summary

Qualifications

• Registered Nurse with a current nursing license.
• BSN preferred.
• Minimum one-year nursing practice or equivalent.
• Hospice, Oncology, or Home Health and/or long-term care preferred.
• Current Tennessee RN License or compact license.
• Hospice certification desirable.

Responsibilities

• Present a professional image and uphold Alive’s standards.
• Provide excellent customer service during patient/facility visits.
• Assess, plan, intervene, and educate with the nursing process for hospice patients.
• Document care at the point of care and update plans of care as needed.
• Conclude visits with encouragement and ensure necessary supplies and contacts are available.
• Complete post-visit follow-up and team synchronization per Alive CARE standards.
