Job summary

Shalom Hospice in Nashville, TN is seeking a dedicated Registered Nurse to plan, direct, and coordinate hospice care in home and community settings, drawing on nursing expertise and community health experience.

Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse, with a License in good standing to practice.
• Minimum of 1 year of experience as a professional nurse within the last three years; or BSN with two years' experience (at least one in public health, home care or hospice).
• Management experience not required; responsible for supervising hospice aides.
• Background checks by regulation required after offer of employment.
• Excellent observation, verbal and written communication skills; basic math skills; nursing skills per competency checklist.
• Must be a licensed driver with a valid automobile.
• CPR certification required (hands-on demonstration preferred).

Responsibilities

• Completes initial and ongoing comprehensive assessments of patient and family to determine hospice needs.
• Provides professional nursing care using the nursing process.
• Writes and initiates plan of care; revises as needed.
• Initiates appropriate preventive and rehabilitative nursing procedures and administers medications.
• Counsels patient and family; provides health care instructions.
• Acts as Case Manager for assigned caseload as directed.
• Participates in on-call duties and ensures equipment/services are available.
• Supervises ancillary personnel and contributes to continuing education.
