Job summary

Advanced Home Health and Hospice in Meridian, Idaho is seeking a Hospice RN Case Manager to coordinate and manage the care of patients in a hospice setting. You will provide skilled nursing services and oversee care plan implementation within an interdisciplinary team.

This full-time position requires strong clinical knowledge, excellent communication, and the ability to work autonomously while ensuring patient comfort, dignity, and alignment with families'' goals.

Qualifications

• Current RN license in the state of practice.
• 1 year preferred hospice experience.
• Minimum of one year of RN experience in hospice or related setting.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and manage time effectively.
• Valid driver''s license and reliable transportation for home visits.
• Current CPR certification.
• Knowledge of regulatory requirements governing hospice care and documentation.
• Compassionate, patient-centered hospice care commitment.
• Experience with EMR systems and documentation preferred.

Responsibilities

• Conduct comprehensive patient assessments to identify healthcare needs and develop care plans.
• Provide skilled nursing care, including pain and symptom management and patient education.
• Coordinate and supervise hospice aides and other healthcare professionals.
• Communicate with patients, families, physicians, and healthcare team.
• Monitor patients'' well-being and comfort during end-of-life care.
• Collaborate with interdisciplinary team to implement care strategies aligning with patient goals.
• Ensure compliance with regulatory guidelines and documentation requirements.
• Provide bereavement support to families and caregivers.
• Participate in team meetings to discuss patient plans and progress.
• Perform other duties as assigned by Hospice Director or supervisor.

Education

RN license (Current Registered Nurse license in state of practice)
