Job summary

Compassus in Louisiana is seeking a Hospice Registered Nurse (RN) Case Manager to provide compassionate at-home care. You’ll evaluate patient needs, coordinate with the care team, and support families while delivering comfort and dignity.

You must be an RN licensed in the state, with at least two years of nursing experience (hospice preferred), a valid driver’s license, CPR/BCLS, and a commitment to building trusting relationships with patients and their loved ones.

Qualifications

• RN license valid in the state you work.
• Minimum 2 years of registered nursing experience; hospice experience preferred.
• Current driver’s license; CPR/BCLS certification.

Responsibilities

• Coordinate patient care with the care team to provide comfort and dignity.
• Build trusting relationships with patients and families.
• Educate caregivers on home hospice care.
• Support goals of care and symptom management in collaboration with the team.
