Job summary

The Care Team Hospice is seeking a Registered Nurse Case Manager to provide skilled nursing and coordinated end-of-life support to patients in their homes and facility settings within Livingston County, MI. You will collaborate closely with the interdisciplinary team to ensure high-quality patient care.

Typical schedule is Mon-Fri 8:00 AM–4:30 PM with on-call rotation opportunities; active RN license and hospice/home health experience preferred.

Qualifications

• RN license active and in good standing.
• Hospice or home health experience preferred.
• Strong assessment and care coordination skills.
• Ability to work independently in the field.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized plans of care
• Provide skilled nursing care in accordance with hospice standards and physician orders
• Coordinate care with physicians, caregivers, and interdisciplinary team members
• Educate patients and families on symptom management, disease progression, and end-of-life care
• Document assessments, interventions, and care plans accurately in the EMR HCHB
• Participate in interdisciplinary team meetings and care planning
• Support after-hours patient needs as part of an on-call rotation when applicable
