Job summary

Halcyon Home LLC in Austin, Texas seeks a dedicated Full-Time Hospice RN Case Manager. The role focuses on comprehensive patient assessments, care planning, and education for patients and families. The ideal candidate is an RN with hospice or home health experience and strong communication skills, capable of working independently in a supportive team.

Qualifications

• Current RN license in good standing with state nursing board.
• Experience in hospice or related healthcare setting preferred.
• Strong clinical and assessment skills.
• Excellent communication and interpersonal abilities.
• Ability to work independently and manage time effectively.

Responsibilities

• Perform initial and ongoing comprehensive assessments of patient needs.
• Develop, document, and revise care plans to ensure quality and continuity of care.
• Educate patient and family about disease process, self-care, end-of-life care, nutrition and care needs.
• Coordinate with the IDG to schedule services based on acuity.
• Provide clinical directions to aides and LPN/LVN to ensure quality care.
• Document progress notes and communicate changes to the physician.
