Job summary

Topkare Hospice is seeking a Registered Nurse to provide comprehensive hospice care to patients in their homes and communities. The RN will assess patient needs, develop care plans with interdisciplinary input, administer medications, and coordinate care with families and physicians.

We value holistic, compassionate end-of-life care and offer strong leadership support, ongoing education, and a collaborative work environment. This is a full-time, in-person role based in San Diego County, CA.

Qualifications

• Registered nurse with current licensure to practice professional nursing in the state.
• Must be a licensed driver with a dependable automobile.
• 1 year of nursing experience in the last 3 years OR a BSN and current RN license; 2 years preferred.

Responsibilities

• Completes an initial, comprehensive, and ongoing assessment of the patient and family to determine hospice needs.
• Provides professional nursing care using the nursing process.
• Develops and revises the care plan with patient and family involvement.
• Administers medications and treatments as prescribed by the physician.
• Acts as Case Manager and coordinates patient care for assigned caseload.
• Maintains accurate clinical notes and communicates with the physician and care team.
• Teaches patient and family self-care techniques and health instructions.
• Ensures a safe environment and continuity of care.
• Participates in on-call duties and quality improvement activities.
