Job summary

New Day Hospice is seeking a Registered Nurse Case Manager to deliver nursing care in patients' homes and coordinate with the Interdisciplinary Group/Team, families, and referring agencies.

You will assess patient needs, develop and monitor plans of care, educate families about end-of-life care, and ensure accurate documentation. Travel locally is required; on-call duties may apply. This role requires compassion and strong clinical skills to work independently within a team.

Qualifications

• Graduate from accredited Diploma/Associate/Baccalaureate School of Nursing.
• Current RN license and valid driver's license.
• Minimum 2 years RN experience in a clinical care setting; hospice experience preferred.
• English reading/writing proficiency; good interpersonal skills; palliation/end-of-life skills preferred.

Responsibilities

• Initial and ongoing comprehensive assessments of patient needs.
• Develop and implement plan of care with IDG team; coordinate with family and referring agency.
• Educate patients and families about disease, self-care, end-of-life care; provide training as needed.
• Ensure documentation, progress notes, and continuity of quality patient care.
• On-call responsibilities and communication of changes to physicians and team.
• Provide clinical directions to Hospice Aide and LPN/LVN to ensure care quality.
