Job summary

Blue Summit Hospice and Palliative Care in Georgia seeks an RN Case Manager Hospice to coordinate comprehensive end-of-life care for patients and families. You will lead assessments, plan development, and interdisciplinary collaboration to meet physical, emotional, and psychosocial needs.

You will communicate with patients, families, and healthcare teams, ensuring continuity of care while maintaining dignity and respect for patient preferences.

Qualifications

• Georgia RN license active and in good standing.
• Minimum 2 years nursing experience; hospice/palliative care preferred.
• Strong knowledge of hospice care principles and end-of-life care practices.
• Excellent communication and interpersonal skills to interact with patients, families and teams.
• Ability to manage multiple cases with attention to documentation.

Responsibilities

• Conduct comprehensive patient assessments to develop individualized hospice care plans.
• Coordinate multidisciplinary team meetings to review and update goals.
• Monitor patient conditions and adjust plans to evolving needs.
• Educate patients and families on disease progression, symptom management and resources.
• Serve as primary contact to facilitate effective communication and coordination.
• Ensure compliance with hospice regulations and quality standards.
• Advocate for patient rights and dignity in end-of-life care.
• Collaborate with community resources to support patients and families.
