Job summary

Blue Summit Hospice in Roswell, GA, is seeking an RN Case Manager Hospice to coordinate comprehensive hospice care plans for patients and families in the Snellville community. You will work with an interdisciplinary team to ensure compassionate, patient-centered care and smooth communication across all providers.

Qualified candidates will hold a GA RN license with at least two years of clinical nursing experience, ideally in hospice or palliative care, and demonstrate strong communication,

Qualifications

• Current and valid RN license in GA.
• Minimum of two years of clinical nursing experience, preferably in hospice or palliative care.
• Strong knowledge of hospice care principles, symptom management, and end-of-life care.
• Excellent communication and interpersonal skills.
• Ability to manage multiple cases with attention to detail and documentation accuracy.

Responsibilities

• Conduct comprehensive patient assessments to develop individualized hospice care plans.
• Coordinate interdisciplinary team meetings to review and update patient goals.
• Monitor patient conditions regularly and adjust care plans as needed.
• Educate and support patients and families on disease progression and resources.
• Serve as the primary contact for patients, families, and healthcare providers.
• Ensure compliance with hospice regulations and quality assurance protocols.
• Collaborate with community resources to enhance patient and family support.
