Blue Summit Hospice in Snellville, GA is seeking an RN Case Manager Hospice to coordinate comprehensive care plans addressing physical, emotional, and psychosocial needs of patients and families. You will act as a liaison between patients, families, the interdisciplinary team, and external providers to ensure seamless communication and continuity of care.

The role requires a current GA RN license and 2+ years of clinical nursing, preferably in hospice or palliative care.

Qualifications

Current and valid RN license in Georgia.
Minimum of two years nursing experience, preferably hospice/palliative care.
Strong knowledge of hospice care principles and end‑of‑life care practices.
Excellent communication and interpersonal skills.

Responsibilities

Conduct comprehensive patient assessments to develop individualized hospice care plans.
Coordinate interdisciplinary team meetings to review and update patient care goals.
Monitor patient conditions and adjust care plans as needed.
Provide education and support to patients and families on disease progression and symptom management.
Serve as primary contact for patients, families, and providers to facilitate communication.
Ensure compliance with hospice regulations and documentation standards.
Advocate for patient rights and preferences while promoting comfort and dignity.
Collaborate with community resources to enhance patient and family support.
