Qualifications

• Graduate of accredited school of nursing.
• Current California RN license.
• Current CPR certificate.
• CA driver's license and insured vehicle in good condition.
• Excellent observation and communication skills per competency checklist.

Responsibilities

• Completes initial and ongoing assessments to determine hospice needs.
• Performs comprehensive physical exams and reviews patient history.
• Conducts home visits and evaluates patient status.
• Initiates and revises care plans with family involvement.
• Administers medications as prescribed and participates in group meetings.
• Maintains clinical notes and supervises ancillary staff as needed.
