Job summary

Anchor Health in the East Bay is seeking a Registered Nurse Case Manager for our Pediatric Hospice Program. The role focuses on coordinating personalized home-based hospice care, implementing the patient plan of care and communicating needs to the interdisciplinary team.

You will collaborate with families and community physicians, provide nursing assessments, and supervise hospice aides while maintaining accurate notes. CA RN licensure and CPR are required.

Qualifications

• Graduate of NLN-accredited nursing program.
• Current CA RN license and CPR certification.
• 1 year of professional nursing experience in the last 3 years, or a BSN with active RN.
• Experience in public health, home care or hospice preferred.

Responsibilities

• Plan, organize and direct hospice care per patient plan.
• Coordinate patient care with Interdisciplinary team.
• Communicate with physicians and family about patient needs.
• Supervise hospice aides; maintain accurate clinical notes.
