The ideal candidate holds a CA RN license, CPR certification, and proven experience in hospice or home health. A valid CA driver's license and reliable vehicle are required.

Qualifications

• Graduate of accredited school of nursing.
• Current RN license in California.
• Current CPR Certificate.
• Minimum of one year of nursing experience within the last three years or a nursing baccalaureate with current RN license; two years preferred, incl. home care/public/hospice.

Responsibilities

• Perform comprehensive patient and family assessments to determine hospice needs.
• Provide complete physical assessment and medical history; conduct home visits.
• Initiate and revise care plans as patient needs change.
• Develop and implement care plans with patient/family participation.
• Administer medications and treatments as prescribed.
• Attend Interdisciplinary Group Meetings.
• Document accurate clinical notes on patient condition and care.
• Supervise ancillary personnel and delegate tasks as needed.
