The Care Team Hospice in New Philadelphia, OH is seeking a compassionate Hospice RN Case Manager to join our team. You will coordinate patient care, manage plans of care, and collaborate with physicians, caregivers, and the interdisciplinary team to ensure comfort and quality of life.

This role offers a supportive environment with on-call opportunities and a focus on end-of-life care. Active RN license and hospice nursing experience are preferred, and familiarity with Homecare Homebase is a plus.

Qualifikationen

• Active RN license required.
• Hospice nursing experience preferred.
• Experience with Homecare Homebase (HCHB) preferred.
• Strong assessment, communication, and care coordination skills.

Aufgaben

• Provide skilled nursing care, assessments, and symptom management in accordance with hospice standards and physician orders.
• Complete scheduled visits, admissions, and clinical interventions with timely documentation in Homecare Homebase (HCHB).
• Respond promptly to patient, family, and caregiver needs, including after-hours and on-call situations.
• Collaborate with physicians, caregivers, and interdisciplinary team to coordinate care.
• Educate patients and families on symptom management and end-of-life care with compassionate support.
