Job summary

Trinity Health At Home is seeking a Hospice RN Case Manager in Livonia, MI, to provide nursing care to hospice clients under physician orders and organizational policies. You will assess, plan, intervene, and supervise patient care and outcomes as the case manager for hospice patients.

You will collaborate with physicians and the IDT/IDG to coordinate services, educate families, and document progress using the agency system. A valid RN license and 1+ year acute care experience are required.

Qualifications

• Graduate of an approved Nursing education program.
• Current Registered Nurse licensure in the state of practice.
• Minimum of one year experience as a professional acute care nurse.
• Home healthcare/hospice experience preferred.
• Exhibits strong communication and interpersonal skills, with emphasis on grief/loss management.
• Must have a valid driver’s license and reliable transportation.
• Commitment to mission and organizational ethics.

Responsibilities

• Inform clients/caregivers about agency services, responsibilities, and rights before care begins.
• Explain/understand Hospice Benefit accurately.
• Develop and implement an individualized Plan of Care focused on outcomes.
• Coordinate care with other disciplines and services for effective care.
• Assess clients using interview and evaluation, applying nursing judgment.
• Report changes to physician/case manager and obtain orders for plan changes.
• Re-evaluate and update Plan of Care based on progress toward outcomes.
• Assess learning needs related to disease, self-care, and end-of-life issues.
