Job summary

Trinity Health At Home Hospice is seeking a part-time, PRN RN Case Manager to provide one-to-one hospice nursing in the Ann Arbor area. The role requires travel to patients' homes across Ann Arbor and surrounding regions as part of a compassionate, patient-centered care team.

Ideal candidates hold a current RN license in Michigan and at least one year of acute care experience, with preference for home healthcare or hospice exposure.

Qualifications

• Current Registered Nurse licensure in the state of practice.
• Minimum of one year experience as a professional acute care nurse.
• Home healthcare/hospice experience preferred.
• Exhibits strong communication and interpersonal skills, with a special emphasis on grief/loss management.
• Must have current Driver’s license and reliable transportation to and from work site.
• Ability to consistently demonstrate commitment to the mission and Organizational Code of Ethics.

Responsibilities

• Provide one-to-one care with patients in their homes.
• Serve as case manager for hospice patients.
• Collaborate with physician and hospice team to manage symptoms and care plans.
• Utilize Home Care Connect and other remote care tools as applicable.
