Job summary

John Knox Village in Lee's Summit, MO seeks a Registered Nurse to provide home and facility-based nursing care under physicians’ orders, collaborate with the IDT, and ensure compliant documentation and care coordination.

Responsibilities include patient assessment, care planning, delegation, triage, education, and participation in QAPI; requires MO RN license, BLS, BSN preferred; competitive benefits and on-site support.

Qualifications

• Combination of education, training and experience from an accredited School of Nursing. BSN preferred.
• Must be a Registered Nurse by Missouri State Board of Nursing or hold a multi-state license that allows practice in Missouri.
• Current Basic Life Support (BLS) certification or ability to obtain prior to patient contact.

Responsibilities

• Provide home and/or facility visits for patient assessment, treatment and support.
• Develop and revise nursing care plans in collaboration with the patient’s physician.
• Delegate nursing care and supervise other team members; ensure delivery of care.
• Maintain and complete accurate documentation and care coordination notes.
