Job summary

Anchor Health in Fremont, CA is seeking a dedicated Hospice Registered Nurse to perform initial and ongoing assessments, develop care plans, administer medications, and participate in interdisciplinary team meetings.

The role requires California RN license, CPR certification, a valid driver's license, and at least one year of nursing experience in home care or hospice. You will travel to patient homes and collaborate with families to meet patient goals.

Qualifications

Graduate of accredited school of nursing.
Current CA RN license.
Current CPR Certificate.
Minimum of one year of nursing experience in home care, hospice, or public health (or two years with BSN).
Must have a valid CA driver license and insured automobile.
Excellent observation and communication skills.

Responsibilities

Completes initial, comprehensive assessments of patient and family to determine hospice needs.
Provides physical assessment and history of current and past illness.
Performs home visits and evaluates patient status.
Initiates and revises the plan of care as patient needs change.
Develops care plans with goals based on nursing diagnosis and family involvement.
Administers medications and treatments as prescribed.
Attends Interdisciplinary Group Meetings and collaborates with the team.
Completes and maintains accurate clinical notes on patient condition and care.
Supervises ancillary personnel and delegates tasks.
