Job summary

Harmony Hospice is seeking an experienced Hospice RN in Chicopee, Massachusetts to provide compassionate, patient-centered end-of-life care as part of our dedicated team. You will coordinate care with the IDG, develop and implement plans of care, document accurately, and collaborate with physicians to address changes in patient condition.

Qualifications

• Graduate from an accredited School of Nursing.
• Licensed in the State of Massachusetts as an RN.
• One year of RN experience; Hospice experience preferred.
• Excellent assessment skills and problem-solving abilities.
• Strong organizational and time management skills.
• Ability to communicate well, both verbally and in writing.
• Reliable transportation and a valid driver's license.

Responsibilities

• Coordinate hospice care with team, patients, and family to meet needs.
• Evaluate effectiveness of hospice services for patient and family.
• Provide professional nursing care using the nursing process.
• Consult with attending physicians and Hospice Medical Directors on changes.
• Coordinate patient services with IDG and contracted services as needed.
• Submit complete, accurate clinical notes in a timely manner.
• Participate in IDG and case conferences.
• Develop and implement the initial care plan with the IDG and maintain the plan.
