Job summary

Pathways Healthcare is seeking a Hospice Registered Nurse to join our patient-centered team in Massachusetts. The role covers Fall River and surrounding areas with a Monday–Friday schedule and full-time status.

The ideal candidate holds an active nursing license, has 2+ years of nursing experience (geriatrics preferred), and is proficient with EMR systems. A valid driver’s license and reliable vehicle are required.

Qualifications

2+ years nursing experience, geriatrics preferred.
Degree in Nursing from an accredited institution.
Proficient with electronic medical records.
Excellent bedside manner and communication skills.
Working knowledge of healthcare laws and regulations.
Maintain current BLS CPR certification.
Valid driver's license and reliable, insured vehicle.

Responsibilities

Assesses and evaluates patient status and initiates plan of care.
Regularly re-evaluates patient and family needs; revises plan as needed.
Develops care plans with patient and family involvement.
Administers medications and treatments per physician's plan.
Completes accurate clinical notes and documents pain/symptom changes.
Communicates with physician and obtains orders as required.
Provides a safe patient environment and attends interdisciplinary meetings.
Participates in on-call duties and ensures equipment and services are available.
Supervises ancillary staff and delegates duties as needed.
Engages in ongoing professional development and quality improvement.
