Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state RN license and valid driver's license.
• Two years RN experience in a clinical setting; hospice/home health preferred.
• Current CPR certification.
• Proficient with computers; Microsoft Office experience preferred.
• Strong interpersonal, communication, and organizational skills; high ethics and compliance.

Responsibilities

• Administer skilled nursing care in clients' homes or residence.
• Coordinate care with IDT and family; manage care plans.
• Educate patients and families about disease process, end-of-life care, and self-care.
• Initiate preventive and rehabilitative nursing procedures.
• Document progress notes and care milestones.
• Coordinate services and refer to other providers as needed.
• Communicate care changes to physicians and staff.
• Assess caregiver ability to meet patient needs; adjust as needed.
• Perform on-call duties as assigned.
