Job summary

VitalCaring is seeking a compassionate Hospice Licensed Nurse (LPN/LVN) to deliver end-of-life care with dignity and empathy. You will work with RNs, aides, social workers, and chaplains to support patients and families in meaningful ways.

Responsibilities include implementing care plans under RN supervision, managing pain and symptoms, educating families, and documenting care in the EMR. A valid LPN/LVN license and CPR certificate are required.

Qualifications

• Active LPN/LVN license in the state of practice.
• Current CPR certification.
• 1-3+ years of nursing experience preferred (hospice, home health, or med-surg).
• Strong clinical judgment, communication skills, and emotional intelligence.
• A genuine passion for patient-centered, end-of-life care.
• Valid driver's license, reliable transportation, and auto insurance.

Responsibilities

• Deliver compassionate hospice care to patients at home or in hospice settings.
• Implement individualized plans of care under RN supervision.
• Provide pain and symptom management and monitor patient response.
• Educate patients and families on disease progression and comfort measures.
• Document care accurately in the EMR and communicate changes to the care team.
• Collaborate with interdisciplinary partners to coordinate holistic support.
