Job summary

VitalCaring Group in the United States seeks a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) to provide direct, compassionate nursing care to hospice patients under RN supervision. You will support families, manage comfort, and deliver dignified end-of-life care in a home-health setting.

This full-time salaried role offers stability, professional growth, and a supportive team culture. A valid LPN/LVN license, CPR certification, a driver’s license, and healthcare experience

Qualifications

  • Graduate of an accredited practical nursing or vocational nursing program.
  • Minimum of three years of nursing experience preferred; home health, community-based, or medical-surgical experience ideal.
  • Current state LPN/LVN licensure.
  • Current CPR certification.
  • Licensed driver with a reliable, insured automobile meeting organizational requirements.

Responsibilities

  • Provide direct patient care in accordance with the state Nurse Practice Act, including specialized duties related to end‑of‑life care.
  • Implement current nursing practice following a comprehensive assessment and the established plan of care.
  • Assess and educate patients and their families/caregivers on diagnosis, care needs, and symptom management.
  • Use and prepare equipment and supplies safely and effectively, consistently adhering…
  • Document all care accurately and timely in alignment with the plan of care.
  • Participate in care coordination and promptly report needs requiring involvement from other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the RN and/or Clinical Care Manager.
  • Participate in ongoing personal and professional development.
  • Contribute to quality assessment and performance improvement (QAPI) activities.
  • Perform other duties as assigned by the Registered Nurse.

Education
