Job summary

Nova Hospice is seeking a Per Diem Licensed Vocational Nurse to provide direct patient care under RN supervision. The successful candidate will implement the plan of care, document actions, and educate patients and families while coordinating with the interdisciplinary team to achieve positive outcomes.

Applicants should hold an LVN license in Texas, have CPR certification, and possess reliable transportation. Hospice experience is preferred.

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• Texas LVN license required.
• Three years nursing experience preferred (hospice/community/home health).
• Current CPR certification.
• Possesses a valid driver's license with insured vehicle.

Responsibilities

• Provide direct patient care under RN supervision per plan of care.
• Implement plan of care initiated by the RN.
• Document accurately and timely per the plan of care.
• Educate patients and family/caregivers about diagnosis and plan.
• Coordinate home health services with other disciplines.
• Use equipment efficiently and safely.
• Participate in professional growth and development.
• Perform other duties as assigned by the RN.
