Job summary

Eternal Faith Hospice seeks an Licensed Vocational/Practical Nurse to provide in-home nursing care across patient ages, under RN supervision. You will participate in creating and executing the Interdisciplinary Plan of Care, document care activities, and ensure compliance with infection control standards while supporting families with empathetic communication.

Two years LVN experience in clinical settings and one year hospice experience are preferred, with a valid LVN license and driver's

Qualifications

• Two (2) years of LVN experience in a clinical setting.
• One (1) year in hospice care preferred.
• Graduate of an accredited Diploma or Associate School of Nursing.
• Current State LVN license and valid driver's license.
• Strong interpersonal communication and adherence to regulations.

Responsibilities

• Under RN supervision, identify patient care needs and document them.
• Plan and implement care per Interdisciplinary Plan of Care with RN.
• Deliver nursing care with infection control measures.
• Coordinate care with the Interdisciplinary Team and family.
• Communicate with physicians involved in patient care; submit required paperwork on time.
• Provide on-call nursing services and travel to patient homes as needed.
