Job summary

AccentCare, Inc. is seeking a dedicated Nurse LVN/LPN for Continuous Care in Hospice. This full-time, on-site role covers multiple counties with a Monday–Friday schedule and a $25.00–$33.00 hourly pay range.

You will provide skilled nursing care, coordinate with the care team, and support patients and families in end-of-life care. Qualified candidates hold an active LPN license, prefer ADN, and have hospice or home health experience.

Qualifications

• Must hold a current Licensed Practical Nurse (LPN) license in practicing state(s); associate degree preferred.
• CPR certification is required for direct patient care where applicable.
• Requires at least one year of nursing experience in hospice, home health, or equivalent patient population.
• Able to work as part of an interdisciplinary team with knowledge of hospice policies and protocols.
• Proficiency in Microsoft Office and EMR systems.

Responsibilities

• Deliver skilled nursing care to patients and families per the interdisciplinary plan of care.
• Coordinate with care team, physicians, and external partners to ensure high-quality continuous care.
• Educate and support caregivers on medication management, personal care, mobility, and end-of-life preparation.
• Serve as key communicator with the Director of Clinical Operations and IDG to address complex issues.
• Provide compassionate support during critical moments and participate in 24/7 care coverage as needed.
• Participate in team meetings, training, compliance and related duties within licensure scope.
