Job summary

Madrona Hospice Care Inc. in San Francisco, CA is seeking an experienced Licensed Practical Nurse (LPN/LVN) to provide direct patient care in home hospice settings under the supervision of an RN. This per diem role offers flexible hours and travel to patient homes within California.

Responsibilities include implementing care plans, educating families, documenting care, and collaborating with the care team to ensure compassionate, high-quality service for patients at the end of life.

Qualifications

• Graduate of an accredited practical or vocational nursing program.
• Active LPN/LVN license in California.
• Minimum 3 years of nursing experience; home health or med/surg background preferred.
• Current CPR certification (AHA).
• Valid driver's license, insured vehicle, and ability to travel to patient homes.
• Strong communication skills and a patient-first mindset.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act.
• Implement and follow individualized care plans established by the RN.
• Educate patients and their families on self-care techniques and diagnosis-related information.
• Document care accurately and in a timely manner.
• Collaborate with the care team - including OT, PT, HHA, and other disciplines - and communicate patient needs to the RN or Clinical Supervisor.
• Use supplies and equipment safely, adhering to aseptic technique.
• Perform other duties as required for patient services.
