Job summary

VITAS Healthcare in Wisconsin seeks an LPN/LVN to provide skilled nursing care to terminally ill patients under the plan of care and applicable nursing standards. You will collaborate with an interdisciplinary team to implement treatments, monitor patient conditions, and document progress.

Responsibilities include medication administration, patient teaching, ongoing assessments, and participation in on-call rotations with flexible scheduling.

Qualifications

• Graduate of an approved Practical/Vocational nursing program.
• Currently licensed to practice nursing in the state where the program is located.
• At least one year of satisfactory nursing experience, at least one year as LPN/LVN in a hospital on an adult medical-surgical unit.
• Equivalent experience may be considered.

Responsibilities

• Provide direct skilled bedside nursing care.
• Assist in the interdisciplinary plan of care for each patient.
• Administer treatments and medications under RN supervision.
• Evaluate effectiveness of treatments and monitor patient responses.
• Document reports in clinical notes and maintain accurate records.
• Maintain a clean and safe environment for patients and family.
• Participate in Nurse LPN/LVN on-call rotation/schedule as assigned.
• Be willing to work flexible schedules, including weekends or rotating shifts.
