Job summary

VITAS Healthcare in West Allis, Wisconsin, seeks an LPN/LVN to join our hospice team providing compassionate, professional care.

You will deliver direct bedside nursing, assist in interdisciplinary care planning, administer treatments under supervision, monitor responses, document thoroughly, and support patient and family education and discharge planning.

Flexible scheduling and commitment to quality end-of-life care are expected as part of our patient-centered approach.

Qualifications

• Graduate of an approved Practical/Vocational nursing program.
• Current Wisconsin nursing license to practice.
• Minimum one year of nursing experience, including at least one year as an LPN/LVN on a hospital medical-surgical unit.
• Equivalent experience may be considered.

Responsibilities

• Provide direct skilled bedside nursing care.
• Evaluate patient/family needs and assist in interdisciplinary care planning.
• Perform nursing tasks under RN supervision, including administration of treatments and medications.
• Evaluate treatment effectiveness and monitor patient reactions.
• Report changes to the nurse, Team Manager or Physician; document in notes.
• Maintain a clean and safe environment for patient and family.
• Maintain accurate documentation on interdisciplinary charts.
• Educate patient/family and assist in discharge planning.
• Participate in ongoing professional growth and education.
• Cooperate with education and research activities and on-call rotations.

Education

Practical/Vocational nursing diploma

Licensed to practice nursing in Wisconsin

One year nursing experience as LPN/LVN on a hospital med-surgical unit

Equivalent experience may be considered
