Emerald Coast Hospice is seeking a compassionate Hospice Licensed Practical Nurse (LPN) to provide bedside nursing care to patients in homes, assisted living, or facility settings in Florida.

You will support the interdisciplinary team, assess patient needs, and implement individualized care plans, reporting changes to the RN. A Florida LPN license, CPR, and a valid driver’s license are required.

Competenze

• Graduate of an accredited Practical Nursing program.
• Current LPN/LVN license in the state of employment.
• Minimum 2 years of LPN experience in hospice, oncology, long-term care, or home health; 3-5 years preferred.
• CPR certification required.
• Valid driver’s license and current automobile insurance.

Mansioni

• Provide direct nursing care to hospice patients at home, in assisted living, or facilities.
• Assist interdisciplinary team with patient/family assessments and care plans.
• Report changes in patient conditions promptly to RN and care team.
• Deliver empathetic care per each patient’s plan of care.
• Collaborate with hospice aides and team members for comprehensive care delivery.
