Legacy Hospice, Inc. in Missouri is seeking a Licensed Practical Nurse (LPN) to join our care team and deliver compassionate hospice services in patients' homes.

You will provide hands-on nursing, educate families, administer medications, and collaborate with RNs and the interdisciplinary team to ensure dignified end-of-life care.

A valid LPN license, at least 1 year of nursing experience (hospice/home health preferred), and a current BLS certification are required.

Qualificações

• Deliver direct nursing care under RN direction per plan of care.
• Administer medications and treatments as ordered.
• Monitor patient condition and communicate changes to the RN.
• Educate patients and caregivers on self-care and meds.
• Assist with daily living activities to promote comfort.
• Maintain accurate clinical documentation and reporting.
• Participate in interdisciplinary team meetings and QA initiatives.
• Provide safe, compassionate end-of-life care.
